These terms are intended to describe conditions for using primestatefit.com and submitting the Free Assessment during the initial assessment and lead-generation launch. The Free Assessment and any future paid Coaching Agreement have distinct purposes. Paid coaching enrollment is not completed through the initial website assessment flow.

Prime State LLC’s Free Assessment and coaching services are intended exclusively for adults aged 18 and older. Individuals under 18 must not submit the Free Assessment or enroll in coaching services.
03 / Description of Services
During the initial website launch, Prime State LLC presents its fitness technology approach, offers a Free Assessment and allows prospective clients to contact the business through WhatsApp and email. Assessment responses are collected using Google Forms and Google Sheets.
Prime State’s approach connects training, nutrition, recovery, daily activity and progress with structured methodology, human guidance and AI-assisted support where used. Descriptions of coaching capabilities do not establish that paid services or every described technology are available through this website. Any future coaching services depend on a separate offering and agreement.
04 / Free Assessment
The website Free Assessment is free and is an initial information-gathering step for adults interested in Prime State. It collects contact information, age range, fitness goals and general responses. A Prime State personal trainer reviews the assessment and may contact the individual about goals and next steps. The assessment is not enrollment in paid coaching and is not a medical evaluation.
05 / Coaching Services and Enrollment
Completing the Free Assessment does not automatically enroll anyone in coaching. Any future paid coaching would require a separate proposal and Coaching Agreement outside the website assessment process. More detailed fitness and health-related information may be collected separately after enrollment, subject to applicable privacy requirements.
06 / Payments, Subscriptions, Cancellations and Refunds
The Free Assessment is free and creates no payment obligation. This website does not sell products or process payments. Any future paid coaching conditions, including pricing, payment, cancellation, renewal, and refund terms, would be presented separately before purchase and governed by the applicable agreement and law.

Prime State provides fitness and nutrition guidance, not medical diagnosis or treatment. The Free Assessment and coaching guidance are not substitutes for individualized advice from a qualified healthcare professional. Exercise and nutrition needs vary, and results are not guaranteed. 08 / AI-Assisted Services and Human Oversight
Prime State may use AI-assisted support to help answer questions and maintain continuity around a client’s plan. AI support complements structured methodology and human oversight; it does not replace human judgment or medical advice. AI-generated information can be incomplete or inaccurate.
09 / Assessment Information
Visitors should provide accurate information when completing the Free Assessment and seek clarification if assessment follow-up is unclear. Submitting the assessment does not create coaching-client obligations. Any future coaching responsibilities would be addressed in a separate Coaching Agreement.
10 / Intellectual Property
Prime State branding and website content may be protected by intellectual property rights. Using the website does not transfer ownership of protected materials.
